Admission Requirements Checklist:
Completed application
form.
Pre-Admission
medical completed and signed by a qualified physician prior to
admission.
Be 18 yrs.
of age or over, and the free from chemical and gambling for (5)
days prior to admission.
Must have no
legal, medical, or other issues which may interfere with the
program.
Is experiencing
problems related to gambling, and expresses a desire to change
present lifestyle.
Does not require
active medical, legal or psychiatric attention.
Needs a safe,
structured environment.
Has completed
all components of referral package including provision of information
regarding transportation.
